Freeswitch is a B2BUA, you shouldn't be needing to use refer at carrier level. The call forwarding is done within the PBX so the forward is just like a regular external call.

How did you install, fusionpbx doesn't use apache it uses nginx? If apache was installed with your distro, start again but remove the apache first.

What happens if you: reload mod_shout from fs_cli, do you see any errors?

Also, you mentioned dialplan, do you know you can just set this in advanced/variables?

The register setting is the same for outbound too, if you set it to tcp all your traffic out will be TCP. Did you stop and start the gateway after you made the changes? You did not need to touch the SIP profiles.

Well, the above looks like you are using a dns challenge rather than an http challenge You need to create the DNS txt records

Are you replacing the external sip/rtp ips on failover? If so and its still not working, before restarting the external profile, flush cache rm -fR /var/cache/fusionpbx/*

Does a sip capture show the wrong IP in the INVITE?

Have you tried restarting the external profile when the failover has happened?

Does anyone use https://fdps.fanvil.com. I didn't know it existed until Dave Jacob pointed me in the right direction. I'm told its not that good though.
