Unitel Voice

I am trying to set up Unitel Voice SIP Trunk thru FusionPBX, but I am confused. Here is what they offer:
A trunk ID and/or Username
A Passcode
A Domain
A Trunk Description

I do not understand where to put these items in the FusionPBX interface.

Further, they say to "Whitelist" a number of IP addresses on UDP and ports 5060-5080. With no other instructions, these I placed into my router's port forwarding list for my servers IP address.

Can anyone help?

They responded that the use of the Port Forwarding in the gateway device was correct. Hope this helps the next person.
