mod_local_stream.c:473 can't open 'filename.wav' next to mod_sndfile.c:281 error opening file

Where filename is an uploaded wave file that is there.

I cannot find where this is trying to be called from and can't find an6y log files to help me from these two scripts (exe's?)

this is happening hundreds of times a second - can anyone give me any pointers on where to look please?

we have changed lots of things, looked through the entire database with omni searching for that filename couldn't find it.

what's odd is that I would expect if it were that file that was missing, it would only error when the on hold music was supposed to be played - but this is hundreds of times every second whatever else is going on...
