SOLVED Fax Server Error - Outbound

Hi
i have installed FusionPBX 5.1.0 just now on Debian 11.
I have created a fax server properly and used correct Caller ID.
I have enabled Fax Queue and started it.
Fax goes to "Trying" Status but never gets sent as i see errors in freeswitch logs. Errors are as below:

ERR] switch_core_session.c:2766 Invalid Application txfax
[ERR] switch_cpp.cpp:1465 [database] can not bind parameter: undefined parameter: fax_result_code
[ERR] mod_lua.cpp:202 ...eeswitch/scripts/app/fax/resources/scripts/hangup_tx.lua:449: attempt to concatenate global 'fax_result_code' (a nil value)


any idea how to fix this?
