Polycom provision 4.0 bug

Hi

I noticed that the polycom 4.0 (mac).sip is missing the following line
<Volume voice.volume.persist.headset="1"/>

I add it manually to my server

I am not sure how to submit to git hub

Slightly unrelated, is there a way to submit a bug/request to the FusionPBX dev team? The "issues" page seems to be disabled on their GitHub page.
