menu disappeared

added a new app, bulk_account_settings this morning then went to Upgrade and attempted menu, permissions, schema, app defaults and I see this

Tried again minus the schema and now I see

Now I've lost the menu. Logged out then back in and still not there. I'll restore a backup from last night if I have to but what's going on here

Cleaning up some domain we use for testing and deleting a domain I get this. What on earth is going on here!!
