Without seeing your configuration it's impossible to know what the issue might be. FS sends early media so there's something else going on. It's going straight to the extension and not a ring group right? Can you post the configuration you have without sensitive info? Also do you have any CLI...

It's probably dead music on hold. Can you explain your trunk settings (screenshots or XML output), and if its going to a ring group or a queue and if MOH is defined in the destinations? Be sure to obfuscate any sensitive info.
