    Actually try the opposite of this and set it to disabled, especially if you're using a self signed cert. It's theoretically less secure as the phone will accept any cert, but for the purposes of troubleshooting I'd give it a try.

    I'm not familiar with FortiGate specifically but usually this is a case of the NAT mapping timing out. I'd look to see if you can change any of the timeout values on UDP mappings, failing that decrease the time on your keep alive messages in the phone config, assuming you've enabled keep alive.

    Could be a codec mismatch. Sometimes a phone might have G722 at the top of its order and won't negotiate down or vice versa. Check the logs.
